Bind: HO3 attestation (modal)

As an agent, I want to upload the HO3 dec page PDF and enter structured fields, with a single checkbox attesting "this is a valid in-force HO3 matching our requirements."
As Althea UW, I want HO3 attributes validated against bind rules — Coverage A ≥ Althea wind Coverage A, insured names match, expiration within 6mo of Althea wind expiration.
As Althea UW, I want HO3-by-carrier patterns logged so I can identify problem carriers for the program over time.

Dec page upload — agent uploads HO3 PDF. No AI extraction at MVP; agent enters structured fields by hand. PAS stores PDF for audit.
Attestation checkbox — agent's representation; backstopped by HO sig at Set A doc #4 ("HO3 Underlying-Coverage Confirmation").

HO3 expiration must be within 6 months of Althea wind expiration; beyond that → UW referral (Frame 21).
Coverage A on HO3 must equal or exceed Althea Coverage A — wind-only is layered on top, not replacement.
Carrier shown as free-text but should be a dropdown of admitted AL HO3 carriers in production (data source TBD per OQ).
AI extraction (OCR of dec page → auto-fill these fields) is Phase-2.
